Continuity of Supports Policy

1. Purpose
To make sure a participant’s support doesn’t collapse if a single worker (including the Director) becomes unavailable — illness, leave, or KindPath ceasing to operate a service line.

3. Policy statement
KindPath’s current scale means real single-points-of-failure exist — this is named honestly rather than assumed away. Every participant’s support plan documents a continuity approach appropriate to their situation.
4. Procedure
- Each participant’s file identifies: their primary worker, a backup contact if the primary worker is unavailable short-term, and what happens if KindPath cannot continue supporting them at all (referral pathway, handover information, timeframe).
- Where a participant relies on a single family-member worker (as is currently the case for at least one participant), the Director maintains enough documented knowledge of that participant’s support needs, goals, and current arrangements to step in or hand over without the participant’s support simply stopping.
- Before any planned change (worker leave, service line changes), the participant is given as much notice as possible and involved in planning the transition.
- For unplanned unavailability (illness, emergency), the Emergency and Disaster Management Policy’s contact procedures apply.
5. Roles and responsibilities
- Director: maintains continuity information for every participant; ensures no participant’s support depends entirely on one person’s availability without a documented backup plan.
