KindPath Collective Inc — NDIS Service Agreement

5. Responsibilities of KindPath

  • Deliver supports as agreed, by screened and inducted workers.
  • Communicate any changes to workers, cost, or availability in advance.
  • Handle the Participant’s information per the Information Management and Privacy Policy.
  • Provide an easy way to give feedback or complain (see Feedback and Complaints Management Policy), including the right to complain directly to the NDIS Commission at any time.

6. Responsibilities of the Participant

  • Communicate their preferences, goals, and any changes to their circumstances that affect support.
  • Provide reasonable notice of cancellation where possible.
  • Treat KindPath workers with the same respect and professional courtesy expected in return.

7. Ending this agreement

Either party may end this agreement with [ ] notice. The Participant can end it at any time without needing to give a reason. KindPath will support a smooth transition per the Transitions (Entry and Exit) Policy.
