Quality Management and Continuous Improvement Policy

1. Purpose
To ensure KindPath actually improves its practice over time based on real evidence — complaints, incidents, participant feedback and outcome data — rather than policies existing on paper only.

3. Policy statement
KindPath treats every complaint, incident, and piece of participant feedback as data, not just as an isolated event to close out. This policy sets the review cadence that turns that data into actual changes to policy, training, or practice — including the data collection work Sam is standing up through the KiNDIS app and the ABC (Antecedent-Behaviour-Consequence) approach being used with participants where formal behaviour support isn’t yet in place.
4. Procedure
- Continuous logging. Incidents, complaints, and (where relevant to a participant) ABC behaviour data are logged as they occur, not batched.
- Quarterly review. At minimum quarterly, the Director reviews: complaint trends, incident trends, any patterns across participants or workers, and progress against this folio’s action items (insurance, registration, worker screening, committee establishment).
- Annual policy review. Every document in this folio is reviewed at least annually, or immediately after an incident/complaint that suggests a policy gap.
- Participant and worker input. At least annually, participants and workers are asked directly (in accessible format for participants) what’s working and what isn’t — not just relying on complaints to surface problems.
- Change log. Material changes to any policy are dated and briefly reasoned in that document’s version history, so the folio shows a genuine improvement trail over time, not just static text.
5. Roles and responsibilities
- Director: runs the quarterly and annual review cycle; ensures it happens even when things are busy.
- Workers: contribute practice observations honestly, including where something in this folio doesn’t match how support actually happens.
