Support Planning and Service Agreements Policy

1. Purpose

To ensure every participant has a clear, co-designed support plan and a service agreement that accurately reflects what will be delivered, at what cost, and under what terms.

2. Legislative and standards basis

NDIS Practice Standards, Module 3: Provision of Supports — Support planning; Service agreements. NDIS Act 2013 (Cth) and Rules on service agreements and pricing.

3. Policy statement

No participant receives ongoing support without a service agreement they understood before signing (or, if verbal/informal by the participant’s choice, a documented record of terms discussed and agreed). Support plans translate NDIS plan goals into concrete, actionable steps — see the goal/action plan approach already used for at least one participant, which is the model for all future participants (per the Director’s stated intent to extend this to everyone KindPath supports).

4. Procedure

  1. Intake gathers what’s needed to plan support safely and effectively (see Phase 2 intake documentation) without over-collecting.
  2. Draft plan and agreement developed with the participant, in accessible language, covering: goals, specific supports to be delivered, frequency, cost per NDIS price guide (or private rate, clearly labelled as such and separate from NDIS-funded items), start date, review date, and how to end the agreement.
  3. Explanation, not just signature. The participant (with a support person if wanted) has the agreement explained in plain language and genuinely has the chance to ask questions and negotiate terms before signing.
  4. Goal/action plans. For each NDIS goal the participant wants KindPath’s help with, a specific action plan is developed with the participant setting out concrete steps, timeframes, and how progress will be tracked — modelled on the existing goal/action plan approach.
  5. Review. Service agreements and support plans are reviewed at minimum every 12 months, at the participant’s plan review, or sooner if the participant’s circumstances or goals change.

5. Roles and responsibilities

  • Director: ensures every active participant has a current, signed (or otherwise documented) agreement and plan.
  • Workers: deliver support consistent with the agreed plan; raise it with the Director if delivery is drifting from what was agreed.
