Transitions (Entry and Exit) Policy

1. Purpose

To ensure participants starting with KindPath, moving between workers, or leaving KindPath’s service experience a smooth, respectful transition — not an abrupt or confusing change.

3. Policy statement

A participant’s exit from KindPath — for any reason, including the participant simply choosing to leave — is handled with the same care as their entry. No participant is made to feel they need a reason to leave, and no participant is left without clear information about what happens to their records and any active supports.

4. Entry procedure

  1. Initial contact and intake per Support Planning and Service Agreements Policy.
  2. Participant given accessible information about KindPath, this folio’s participant-facing rights documents, and how to give feedback or complain, before or at the start of support.
  3. First few sessions treated as a settling-in period, with an explicit check-in on fit and preferences.

5. Exit procedure

  1. Exit can be initiated by the participant, KindPath, or arise naturally (funding ends, goals achieved, participant moves region).
  2. Where KindPath initiates ending a support (e.g. cannot safely continue, or a service line ends), the participant is given as much notice as possible and support to transition to another provider, including a warm handover if the participant wants one.
  3. Records are retained per the Records Retention and Disposal Schedule; the participant is told what happens to their information after exit and can request specific information be shared with a new provider (with consent) or not retained beyond the legal minimum.
  4. An exit is not treated as a failure to be minimised in reporting — it’s logged, and if a pattern of early exits emerges, that’s reviewed under Quality Management and Continuous Improvement.

6. Roles and responsibilities

  • Director: ensures every exit includes a proper handover and respects the participant’s choice, whatever the reason.
